what you need to know
Please arrive 5-10 minutes early to your appointment to complete patient paperwork. If you are a new patient, it's helpful to bring a copy of your most recent prescription from your previous order.
If you have vision insurance, please bring a copy of your card. Please call the store 24 hours before your appointment to verify your benefits.
